Shoulder Referral Form

Please see the attached shoulder problem referral form. Appointments will be held at St. Paul's Hospital in Vancouver, BC.

 

​This if for patients residing in British Columbia only (ie have an active MSP number).

 

Don't worry about specifically requesting an appointment at the Rapid Access Shoulder Clinic - we will triage the patients as necessary.

 

Again, this is still in a trial phase so please provide us any feedback you have to the process.
